Cremation Forms:

• Form A - Application for a cremation
• Form B - Certificate of Medical Practitioner
• Form AB - Certificate in relation to pacemakers and other biomechanical aids

completed forms may be submitted by email to interments@ncc.govt.nz or handed in at our customer service Centre, located at 110 Trafalgar Street.
